In essence, care at home is needed when your loved one requires assistance with any activities of daily living while remaining in the familiar surroundings of their own home and continuing to engage in their community.

Assisted Living in public or private apartments where Care Specialists stay-in to provide Home Personal Care services for long term residence.

Assisted Living in cluster facilites which offer community living for those in a similar season of life as long term residence. A centralised Care Team including Care Specialists, Nurses and Physicians will provide care.

Assisted Living in public or private apartments where Care Specialists stay-in to provide Home Personal Care services for short term residence in support of giving the caregiver respite to tend to your own need for rest and recovery.

Ageing-In-Place where Care Specialists stay-in to provide Home Personal Care services in your loved one’s residence for the long term.

DO YOU HAVE A MEDICATION ASSISTANCE PROGRAM?

GROW offers a medical assistance where the Care Specialist
1. Assists with medications that have been prepacked by the client and/ or caregiver and/ or appropriate healthcare professional
2. Refers to the accompanying written instructions from the client/ caregiver/ healthcare provider or institution.
3. Ensures that the right medication pack is served to the client at the indicated time; and
4. The date and time of assistance shall be documented in the client’s file as soon as the client is assisted.

If for any reason the client fails/ refuses to consume the medication, the Care Specialist shall document this in the client’s file, escalate where necessary, and notify the client/ caregiver.

Signoff of the Medication Indemnity Form by the Client/Next-Of-Kin (where applicable) is required.

HOW IS MY LEVEL OF NEEDED CARE DETERMINED?

Our physicians and care team will evaluate your current condition and assess your medical records before your Care Specialist is sent to determine what level of care is best suited for them and you as the caregiver. We will seek to understand their personality, preferences and daily habits to create a care plan that is just right for all. The Individualised Care Plan (ICP) outlines your loved one’s care strategy and is revised regularly to respond to changes. We already have a short online Care Questionnaire to assist you in understanding the types of care that may be required.

HOW MANY TIMES CAN WE VISIT?

There are no restrictions as your loved ones are living in their own homes.
